Records Request Policy

Because of the time involved in reviewing your chart, copying and then mailing or faxing medical records, our office does charge for this service.

We charge for copies of medical records in the following manner:

10 pages or less = $25.00
11-20 pages = $1.00 per page + .50¢ each additional page
+ $15.00 handling fee
+ postage (where applicable)

To request a copy of medical records please download and complete the Request Form and either mail via standard post service or bring it into the office. Do NOT email the completed form - they will not be accepted.
