Refill Policies

To avoid discrepancies in prescriptions we generally refill prescriptions during office visits. We try to prescribe enough medication until your next appointment. Before your medication runs low, please call us for an appointment.

If you change pharmacies or prescription plans that require us to rewrite current prescriptions, there will be a $10 charge due from you. If we have to pre-certify drugs on your prescription plan there is a $15 fee due prior to our making that call; however, our phone call does not guarantee that your plan will provide coverage.

If necessary we will refill prescriptions between visits but we charge $10-$15 for this extra service. If you need a renewal between visits, you can call your pharmacy and have them fax the request to us at 330-665-5133; or you can fill out the prescription refill request form and submit the request via fax to us. You can also contact your pharmacy on the Internet if you use CVS, RiteAid, Walgreen's, Giant Eagle or Tops Markets.

Please remember to have your prescriptions refilled at your office visits. Any prescriptions that we mail to you will have a $10.00 handling charge and will be billed directly to you.
